We thank you for your interest in the activities and initiatives of the Floorball Club Milano Amateur Sports Association.
Before releasing your personal data, we invite you to read the following information concerning the processing to which your data will be subjected, in full compliance with the European Regulation for the protection of personal data n. 679/2016 (articles 13, paragraph 1, e 14, paragraph 1), of the Legislative Decree. n. 196/2003 of the Code regarding the protection of personal data, of the Legislative Decree. n. 101/2018.
The Amateur Sports Association Floorball Club Milano has in fact at heart the protection and protection of the personal data of all those who interact with it..

LEGAL BASIS OF THE PROCESSING
To process your data we must be legitimated to do so. This legitimacy can originate from:
• Your consent
• the association contract with the Floorball Club Milano Amateur Sports Association;
• the legitimate interest of the Floorball Club Milano Amateur Sports Association;
• Authorization no. 3/2016 to the processing of sensitive data by associations and foundations of the Guarantor Authority;
• the law;

DATA PROCESSED
Personal data of a common nature provided by you through the registration and membership form: name and surname, address, region, Nationality, email, telephone number, cellphone number, tax code or VAT number, place of residence or domicile, card number, the image of your face or your person.
Particular personal data: data provided by you through the presentation of the medical certificate of fitness for competitive sports. We would like to point out that the only health information that will be dealt with is related to "suitability" or "unsuitability" for sporting activity.

PURPOSE OF THE TREATMENT
The collected data will be used for the following purposes, all strictly and exclusively related to the statutory purposes of the Floorball Club Milano Amateur Sports Association:
• release information to you;
• allow you to join the Floorball Club Milano Amateur Sports Association;
• allow you to request and obtain the membership card;
• allow the stipulation of the mandatory insurance policy by registering with an EPS (Sports Promotion Body);
• allow you to participate in the Association's activities, of the Italian Unihockey Floorball Federation (tournaments, demonstrations, races etc.) and CONI;
• allow the Association to become affiliated with the Italian Unihockey Floorball Federation and with CONI;
• fulfill contractual obligations with the Italian Unihockey Floorball Federation and CONI;
• carry out, with prior consent, advertising activities, commercial and marketing;
• perform statistical analyzes;
• defend the rights of the Association.

NATURE OF THE ASSIGNMENT
You can and indeed must freely decide to give us your data e, when required, to express consent to their treatment. The provision of common and particular personal data is a contractual obligation and a necessary requirement to join the Association.
Unfortunately, if you do not provide all the requested data, we will not be able to provide you with associates or cards.

PERIOD OF STORAGE OF PERSONAL DATA
Your personal data will be kept for a limited period, connected to the purpose for which they were collected and in any case no later than the time necessary to achieve the purposes related to the statutory purposes of the Association.
Retention times are, without prejudice to any legal obligations and any legitimate interest of the Association are the following:
• common personal data: per 10 years from the termination of the association contract;
• particular personal data: per 5 years from the termination of the association contract;
• newsletter: until the consent is revoked;
• information request: 12 months; At the end of the term, your data will be irreversibly deleted or otherwise made anonymous.

PLACE OF TREATMENT
We will process your personal data at our office. However, for organizational and security reasons we also use external IT service providers (for example for site hosting service, the cloud service, etc.) that we have appointed as data processors.
All our suppliers are qualified to ensure the most adequate level of safety and are based in Italy or in any case within the European Union. Your data will not leave Europe. METHOD’ OF TREATMENT
The processing of your data is carried out both on paper, both through the use of electronic tools. Your data will always be:
• processed lawfully and fairly;
• collected and recorded for specific purposes, explicit and legitimate, and used in other processing operations in terms compatible with these purposes;
• correct or, in any case, corresponding to the information provided to us;
• relevant, complete and not excessive in relation to the purposes for which they are collected or subsequently processed;
• kept in a form that allows the identification of the data subject for a period of time not exceeding that necessary for the purposes for which they were collected or subsequently processed.

SECURITY MEASURES
The processing of personal data is a serious business for us. Your data is our data and we want to protect it in the same way. For this reason we have adopted adequate and suitable technical and organizational security measures to minimize the risk of destruction, of loss, of illicit or incorrect use, unauthorized access or treatment that is not permitted or does not comply with the purposes for which they were acquired.

COMMUNICATION OF PERSONAL DATA
In some cases we will necessarily have to communicate your common and / or particular personal data, as well as our staff, to certain external parties that perform, also on our behalf, all related or instrumental activities.
In particular, the external subjects to whom we will communicate your common personal data are, for example: the accountant, the tax advisor, the newsletter service provider.
In the event that the Association joins the Italian Unihockey Floorball Federation and you decide to participate in its activities, we may have to communicate your personal and particular data to the Italian Unihockey Floorball Federation., with headquarters in Piazza Napoli n. 35, 20146, Milano .
Taking into account the obligation to take out an insurance policy, we will have to communicate your personal and particular data to EPS (Sports Promotion Body) chosen.
We may also disclose your personal data to the International Floorball Federation, based in Lausanne, Svizzera, registered with the Chamber of Commerce of the Canton of Vaud, in the case of international competitions.
These individuals are all qualified and, having been appointed by us as external managers for the processing of your personal data, they will not be able to make any use of your data other than that for which they have been entrusted.
We then inform you that, in other cases, your data may be disclosed to other subjects, entities or authorities whose disclosure of your personal data we are required by law or regulation.

DISCLOSURE OF PERSONAL DATA
Your personal data will not be disseminated (for example, published on the internet, included in the newsletter, facebook pages, etc.), unless you give us your express consent. If you have given us your consent, we will disseminate your image through remote means of communication (television, internet, social network, etc.) and not (message boards, scoreboards etc.), and your common personal data (Name, surname, category, sex, age, Nationality, membership club, statements made) in compliance with the relative authorization you have issued to us.

TRANSFER OF DATA ABROAD
Your personal data will not be transferred to non-EU countries nor is it our intention to transfer them.
However, the dissemination of images through social networks could involve the sharing of some common data collected with service providers. For the transfer of such data, the consent of the interested party will not be required in accordance with the provisions of the Italian and European authorities (Decision No.. 1250/2016 – Privacy Shield).

RIGHTS OF THE INTERESTED PARTY
Your rights are very important to us and we would like to list them all for you. You must know that you can always request:
• access to your personal data and information relating to them, as well as request a copy;
• rectify inaccurate data or integrate incomplete ones;
• request the deletion of personal data concerning you;
• request the limitation of the processing of the personal data you provide;
• request and obtain – in the event that the legal basis of the processing is the contract or consent, and the same is done by automated means – personal data in a structured and machine-readable format, also in order to communicate such data to another data controller (c.d. right to the portability of personal data);

• withdraw consent to data processing at any time. The treatment based on consent and carried out prior to the revocation of the same retains, in any case, its lawfulness.
• ban us, in the case of your untimely death, to let those who have an interest of their own exercise, or acts on your behalf, as an agent, or for family reasons worthy of protection, to exercise, limited to the information society services we offer you, the rights listed above.
